Lamprey Health Care, NH’s oldest Community Health Center with sites in Newmarket, Raymond and Nashua, is currently seeking a Quality Improvement Manager to lead our quality improvement initiatives while supporting our overall vision of a Patient Centered Medical Home. Lamprey Health Care is a learning organization that prides itself on embracing change, and is recognized by NCQA as a level 3 Patient Centered Medical Home providing primary health care to over 16,000 patients annually.
The Quality Improvement Manager will lead, manage, and facilitate the accomplishment of Lamprey Health Care quality programs, including Meaningful Use and the Medical Home initiative and preparing clinical alignment for value –based reimbursement and working with Clinical Director and Administrative leadership for data needs.
Responsibilities include data for population-based care – identifying gaps in care, predictive analytics, works with IT and provider staff for EHR improvements, attend IDN and other meetings around the state, staff education and training about quality measures and results, coordinating process improvement throughout the departments for the organization, and participating in corporate quality improvement initiatives.
The Quality Improvement Manager will facilitate “evidence-based” clinical practices and report performance through standardized measures; Ensure processes to concurrently track quality initiatives, state and federal clinical metrics and provide outcomes feedback; Coordinate an integrated “dashboard” of clinical outcomes, critical interventions, cost, utilization, and satisfaction; and, fulfill the Lamprey Health Care philosophy, that managers actively coach, engage, and support the staff to achieve results.

Education: Bachelor’s degree in health care related field required; R.N preferred or related medical experience. This position requires: seven to ten years of health care experience, preferably in a community health care organization; the ability to track, trend and analyze statistical information; and knowledge of clinical service level benchmark standards; knowledge of electronic health record, of some coding and WEBI programing.
